Face transplant workshop

Face Transplant & Vascularized Composite Allotransplantation (VCA) Workshop
The Hands-on Cadaver Workshop on Facial Vascularized Composite Allotransplantation (Face Transplant Workshop) is a high-level surgical masterclass jointly organized by the Department of Plastic & Reconstructive Surgery, Craniofacial Surgery, and the Department of Anatomy. Representing the pinnacle of modern reconstructive surgery, full and partial face transplantation requires synchronized donor procurement, meticulous facial nerve branching dissection, microvascular pedicle isolation, and structural osteosynthetic insetting.

This masterclass is designed for plastic surgeons, craniomaxillofacial surgeons, microsurgeons, and reconstructive fellows, offering dedicated cadaveric workstations, surgical loupes, high-speed bone cutting consoles, micro-instrumentation, and direct mentorship from experienced reconstructive surgeons and clinical anatomists.

Key Highlights & Dissection Goals
Delegates perform live knife-to-skin dissections simulating donor facial allograft harvesting and recipient bed preparation, mastering facial artery/vein skeletonization, superficial temporal vessel isolation, facial nerve trunk and branch preservation, and sub-SMAS plane elevation.

  • VCA Surgical Masterclass – Complete procedural training on partial and total facial composite allograft harvesting
  • Neurovascular Pedicle Mobilization – Facial artery/vein, superficial temporal vessels, and main facial nerve branches (CN VII)
  • Plastic Surgery & Anatomy Collaboration – Seamless integration of macroscopic anatomy and microvascular reconstructive technique
  • Small Batch Excellence – Strictly limited registration ensuring extensive individual station time and table proctoring
  • Structural Inset Mastery – Rigid osteosynthesis of maxilla/mandible components and aesthetic red-line mucosal restoration
All cadaveric dissections are carried out adhering strictly to ethical guidelines and institutional anatomical protocols, offering surgeons realistic tissue glide planes, fascia elevation feedback, and complex pedicle length optimization.
Structured Hands-on Dissection Curriculum
The program bridges facial vascular topography and craniofacial anatomy with operative case planning, live surgical prosections, and comprehensive cadaveric harvesting.
The curriculum systematically advances from surface incision design through cervical vascular access, parotidectomy-level facial nerve trunk tracing, sub-SMAS full facial mask elevation, bony osteotomies (Le Fort and mandibular segments), to simulated recipient insetting.

Stepwise Workshop Matrix

Session 1: Cervical Access & Main Vessels | Session 2: Facial Mask Elevation | Session 3: Osteotomies & Allograft Insetting
Preauricular & coronal incision design | Sub-SMAS elevation & retaining ligament release | Le Fort I/II and anterior mandibular osteotomies
Facial artery & anterior facial vein skeletonization | Parotid gland dissection & facial nerve main trunk isolation | Suture neurorrhaphy of CN VII branches & sensory V2/V3
External carotid artery & thyrolingual trunk tracing | Periorbital, nasal cartilaginous, and perioral release | Microvascular anastomoses & tension-free allograft inset

Core Surgical Modules & Syllabus
Guided by experienced reconstructive and craniofacial faculty, delegates perform sequential operative dissections simulating complete allograft harvesting:

Cervical crease incision, subplatysmal flap development, and skeletonization of the facial artery and facial vein at the inferior border of the mandible. Proximal tracing to the external carotid artery and internal jugular vein system.
Identification of the main trunk of the facial nerve at the stylomastoid foramen using standard landmarks (tympanomastoid suture, tragal pointer, posterior belly of digastric). Meticulous anterograde dissection of temporal, zygomatic, buccal, marginal mandibular, and cervical branches.
Coronal, preauricular, and submental incision planning. Elevating the facial composite allograft in the sub-SMAS and subperiosteal plane over the zygoma, releasing zygocutaneous and masseteric retaining ligaments while preserving mimetic musculature.
Techniques for Le Fort I, II, or III osteotomies and anterior mandibular osteotomies using reciprocating saws to incorporate midface and jaw skeletal components into composite chimeric allografts.
Receptor bed preparation, plate osteosynthesis, tension-free red-line mucosal matching, micro-neurorrhaphy of motor (CN VII) and sensory (infraorbital, mental) nerves, and end-to-end microvascular anastomoses.
Faculty from the Department of Anatomy demonstrate high-resolution cross-sectional facial spaces, superficial musculoaponeurotic system (SMAS) continuity, and relational vascular anatomy.
Eligibility & Workstation Infrastructure
Designed for reconstructive plastic surgeons, head and neck oncosurgeons, and craniofacial fellows seeking mastery in complex composite facial reconstruction.
Each dissection table receives comprehensive instructions regarding continuous irrigation during osteotomies, safe micro-adventitial handling, and SMAS flap hydration.
